Tom Boucher 1e76459813 fix: replace ambiguous double-question in discussion reflection step (#1226) ... The plain-text fallback for depth verification asked 'Did I capture that correctly? Anything I missed?' — two yes/no questions where a short affirmative maps to opposite intents (yes = confirmed vs yes = missed something). Changed to: 'Did I capture that correctly?
